Who is Eligible for Benefits
If you are an active regular employee with at least 30 scheduled hours per week you are eligible for the benefit plans. For new hires, or those who are newly eligible, benefits become effective the first day of the month following date of hire.
Legal spouses who are eligible for medical insurance coverage through his or her employer are not eligible for the Company’s medical plan. Employees who elect to enroll a spouse on the medical plan will be required to complete an affidavit confirming the spouse’s medical coverage status.
Eligible dependents may also participate. Eligible dependents include your legal spouse and children. Children under age 26 are eligible for the medical, dental, vision, voluntary life and AD&D, and supplemental plans.
If you have already verified your dependents, you will not be required to supply new documentation to verify they are eligible dependents. New hires have 30 days from their date of hire to submit documentation.
Making Choices
Eligible new hires are required to elect or waive coverage within 30 days of their date of hire. Employees failing to elect coverage within 30 days of their date of hire will be deemed to have voluntarily waived the respective coverage for the entire year. Once made, elections are irrevocable and remain in effect for the plan year unless a “Qualified Status Change” occurs.
Qualified status changes include: marriage, divorce, legal separation, death of a spouse or dependent, birth or adoption of a child, your spouse obtaining new employment or terminating employment, changes in health coverage by your spouse’s employer, or your dependent no longer qualifies as an eligible dependent. To elect a change, contact the Company’s Human Resources Department.
